Registered Massage Treatments And Sessions:

Invoices will be provided and are accepted by most Extended Health Benefit Plans * Consult your plan.
Visit your doctor first for massage clearance and ask for a * massage prescription (note) to be submitted to your Health Care Provider for eligible reimbursement under the terms of most Health Care Plans. For you to claim the massage on your work benefits a * physician’s prescription is required that is valid. Please see your Health Care Plan for which you are entitled.
Create A Direct Billing Account AT Sunstone Registered Family Massage
Clients wanting to create a Direct Billing Account with us must first fill out consent forms that are legally required by the eligible insurance companies. It will take 2 business days to validate and set up your direct billing account after we receive the completed consent forms before we can bill insurance directly.
All clients will be contacted directly once their account is complete, verified and ready to be used.
We can email out the blank forms but will not accept completed forms via email to protect client privacy & security.
